In the Horitsuba verse, the twins had to live separated for years since Yuui lived in Italy.
The separation was due to the death of their parents in a tragic accident (well actually that's the story me and ~ChiakiAkito made up in the RpG) and they suffered a lot for this.

Of course, they were allowed to make phone calls and write letters, but if you are used to live really close to someone - especially your twin, with whom you have a veeery strong bond - this will surely mean you miss him a lot, when you are apart.
"Can you hear me" because they're wondering "can you feel my pain? Do you feel lonesome like I do, tonight?"

Nell'Horitsubaverse, i due gemellini sono stati costretti a vivere separati per anni, perchè Yuui era stato portato in Italia.
La separazione era stata causata dalla morte dei loro genitori in un drammatico incidente (o almeno, questo è ciò che le menti mia e di ~ChiakiAkito hanno pensato nel GdR) e i bimbi ne hanno sofferto molto.

Ovviamente, potevano sentirsi al telefono e spedirsi lettere, ma quando si è abituati a vivere a stretto contatto con una persona - specialmente un gemello, con il quale sicuramente si ha un rapporto molto stretto - significa che in ogni caso se ne sente molto la mancanza, quando si è lontani.
"Puoi sentirmi" perchè si stanno chiedendo: "puoi sentire cosa provo? Ti senti anche tu solo, stanotte?"
